SACSA Student Assessment Checklist (PDF) These forms have been made with the South Australian Curriculum Standards & Accountability Framework in mind, though hopefully they can be modified to help other teachers also. The Unit Planner is designed to be a simple Cover Sheet (the kind that Principals like to see) on the front of your programme. The Student Assessment Checklist I like to use while a unit is running, as it allows me to conference, observe and give feedback about a student's progress. When the unit is completed I can add comments, there is an opportunity for student self assessment, and the sheet can be put into student learning folders.
