Before Championships

• Duties of Regional Coach - to be read before completing the nomination form
• Responsibilities of Regional Team Officials - to be read before completing the nomination form
• Nomination Form
• Application for Approval of Duties - to be completed after appointment of the nominated position
• EFT Details Form - allows for allowances to be deposited into your bank account. Only needs to be completed if PSSB does not already have your details or your details have changed
• Team Information Sheet - to be completed as soon as possible after appointment. To be handed in to PSSO by end of Week 1, Term 1.
• Team Sheet - to be submitted within a week of regional championship
• Training Sessions Form
• Risk Management Checklist - Event Venue - to be completed for trainings and trial matches
